As a Wellbeing Practitioner, you’ll play a critical role in helping veterans and their families rebuild stability, confidence and direction, through structured, trauma-informed, evidence-based support.

The Opportunity:
You’ll deliver high-quality case management within a professionally governed framework, combining clinical thinking, compassion, and real-world problem solving. This is not passive support work, this is structured, accountable, outcome-focused practice.

You will:
  • Conduct holistic psychosocial assessments
  • Develop individualised, strengths-based care plans
  • Support clients through complex health and social systems
  • Coordinate care across mental health, housing, financial and community services
  • Manage a caseload of 15–20 clients (complexity-based)
  • Identify and respond to risk and high-complexity situations
  • Deliver short- and long-term interventions that create sustainable change
Who You’ll Support Veterans and their families navigating:
  • Mental health challenges
  • Social isolation
  • Financial hardship
  • Transition from Defence life
  • Trauma and complex life circumstances
This is meaningful, complex work requiring sound judgement, resilience and strong professional boundaries.

What We’re Looking For Essential:
  • Degree in Human Services, Social Work, Psychology, Health Sciences (or similar)
  • Eligible for professional membership/registration (AASW, AHPRA, PACFA etc.)
  • 2+ years in case management, wellbeing or psychosocial support
  • Strong capability in trauma-informed and strengths-based practice
  • Experience conducting assessments and structured care planning
  • Excellent communication and documentation skills
  • Ability to work autonomously within governance frameworks
  • Driver’s license
Highly Regarded:
  • Experience with veterans, Defence or trauma-affected cohorts
  • Knowledge of health, mental health and social service systems
  • Strong clinical reasoning and risk management capability
