We’re recruiting on behalf of a growing, values-led organisation delivering tailored support to individuals with complex psychosocial, cognitive, and physical needs within the NDIS space. This is not a clinic role — it’s real, relationship-based work that meets people where they are.
You’ll provide outreach and telehealth-based support to clients experiencing psychosocial disability, trauma, housing instability, domestic violence, family systems involvement, and neurodivergence. With a strong commitment to neuroaffirming practice, you’ll offer a blend of structured appointments and responsive care to help clients navigate services and achieve meaningful goals.
Your key responsibilities will include:
- Deliver psychosocial assessments, care planning, and case management
- Provide trauma-informed support in areas like housing, mental health, family safety, and more
- Build strong therapeutic relationships in community or home settings
- Manage documentation and reporting in line with NDIS and AASW standards
- Maintain 5 billable hours per day (includes travel), with flexibility for admin and case complexity
- Collaborate with a multidisciplinary team while managing your own diary
- A degree in Social Work and eligibility for AASW membership
- At least 2 years’ experience supporting vulnerable or diverse clients
- A calm, organised, and grounded approach to dynamic work
- Strong time management and clear communication
- Lived experience of disability, trauma, or system navigation is highly valued
- Confidence working independently while staying connected to a supportive team
- Flexibility: Hybrid work model with autonomy to structure your week
- Location: Hybrid (office based near Slacks Creek, QLD + outreach + telehealth)
- Permanent Full-Time or Part-Time (3–5 days/week, flexible)
- Remuneration: Up to $109,000 + super (pro-rata) + mileage reimbursement
